This 162-bed facility opened in 1998 and provides quality care to people from all faiths and walks of life in a caring, Christian atmosphere.

Rehabilitation and short-term care residents are cared for on their own special wing so they receive the privacy and specialized nursing care they need. Our rehabilitation center and aquatic therapy pool are located adjacent to that wing and are easily accessible to Outpatient Rehabilitation patients as well. We also provide long-term skilled nursing care, specialized Alzheimer's and dementia care, and respite care.

Our residents receive the highest quality medical, nursing and personal services, supervised by a primary physician. Licensed gerontologists, consultants in dentistry, podiatry, ophthalmology, psychiatry and surgery provide services regularly. Social, nutritional and pastoral care, all essential to the individual's well being, are also provided. Registered nurses are on duty 24-hours a day.

At GreenField Health & Rehabilitation Center every resident benefits from an individualized plan of care developed and carried out by an interdisciplinary team that meets regularly to discuss progress and update the plan of care as necessary. The team is comprised of the resident and his or her family members as well as our medical director; physical, occupational and speech therapists; and nursing, activities, nutrition and social services staff. If it is determined that a resident is able to go home or to a different level of care, our social services staff will help with discharge planning and arranging for community services and special equipment that might be necessary.
